Mother– Diabetes, Alzheimer’s disease. Died 2 years ago at age 87.

Born in South Carolina. Moved to New England age 28 with husband. Worked in shoe factory for 20 years, then in school cafeteria. Retired at age 65. No tobacco or alcohol use.
Had been living with her son and daughter-in-law, providing care for her granddaughter, until 12 months ago when she moved in with her daughter as she needed more care.
Nursing intake assessment reveals that Ms. Green describes herself as a very religious and spiritual person and has been an active member of local Pentecostal Church for past 18 years.
