Medical History

After an abnormal mammogram, Ms. Green was diagnosed with breast cancer 5 years ago by needle biopsy. She underwent lumpectomy and radiation therapy, followed by treatment with Tamoxifen. Recurrence of her cancer was detected 1 year ago, when she presented with back and hip pain and found to have skeletal metastases. She was treated with chemotherapy regimen of taxane plus capecitabine. 6 months later, she was found to have progression of skeletal lesions and liver metastases. Chemotherapy was changed to her current program which includes an anthracycline.

Past medical history:

Diabetes mellitus, hypertension, TAH/BSO (fibroids), carpal tunnel release.

Medications:

Amlodipine 10 mg QD; HCTZ 25 mg QD; Glimeripide 4 mg BID; oxycontin ER 10 mg BID; Oxycodone 5 mg Q6 hours prn breakthrough pain; Colace 100 mg BID.

Review of Systems:

Loss of appetite, constipation, dry mouth, generalized weakness and fatigue.
