Discharge Plans

The social worker facilitates home hospice services and makes arrangements for follow-up with Mrs. G and her children. The chaplain contacts the pastor at Mrs. G’s church. A follow-up appointment is made in 2 weeks with Dr. Richards.

Discharge medication plan:

  • MS Contin 60 mg Q12 hours.
  • MSIR 15 mg Q1-2 hours prn breakthrough pain.
  • For constipation: colace 100 TID, Senna 2 tablets QD, and Glycolax 18 gm in 8 ounces of water QOD.
  • Because of decreased po intake, Glimeripide dose reduced to 2 mg QD.
  • Because of presentation with dehydration and anticipated low po intake, HCTZ discontinued. Amlopidine continued for hypertension.
  • Per patient request for medication for anxiety: lorazepam 0.5 mg TID prn anxiety.
